By completing this form, you agree to the following conditions

If selected, you are required to pay $350 per season as your player fee. This fee is due upon acceptance to the team. Payment plans will not be accepted.

If selected, you are expected to attend all practices unless prior communication with the coach occurs and approval is granted

All photots and videos obtained during the tryout are the sole property of Kokomo Vipers FC and availble for use at any time by Kokomo Vipers FC for promotional purposes

If selected, you are expected to attend all games during the season. Games are played on Saturdays or Sundays. Some out of state travel and hotel accomdations are required and paid for by Kokomo Vipers FC

Tryout is at your own risk. The Kokomo Vipers FC is not responsible for injury or illness

Players are not entitled to any compensation for use of photos and videos

If selected, you must pay the UPSL registration each season. This is separate from the club fees.

If selected, you understand you are responsible for securing your own housing and your own transportation to and from practices/games without expectation of reimbursment.

You understand, earning a spot on the team does not guarantee a spot on the weekly play roster.

You understand this is a semi-professional league and players are expected to conduct themselves in a manner that fits that level of play.
